[09:30, 29/04/2022] Riaan Mulder : Good morning, I have 14 yr old male cheetah patient with diarrhoea, melaena, priapism (more like a penile paralysis), SDMA 15, Urea 13. ALP low, rest unremarkable. Urine normal, prostate not palpable. Apart from the gastritis, ulceration, why would we have penile paralysis (hanging out)? [10:28, 29/04/2022] Henry Labuschagne: Priapism or paraphimosis ? Post a picture please? [11:08, 29/04/2022] Riaan Mulder H, this is probably not either, sheath is not swollen or inverted, it is not really an erection. I get the impression that is might be neurologic, although no impairment of hindquarters, and spinal X-ray N/A [11:12, 29/04/2022] Willem Burger: This is also not normal! Fungal origin, Cryptococcus or endoparasites? [11:30, 29/04/2022] Henry Labuschagne: Tip of penis looks ulcerated / superficial necrosis. Apply acriflavine / glycerine mixture and Vaseline. [11:37, 29/04/2022] Adrian Tordiffe: Do you have haematology results or a haematocrit? The BUN in this cheetah is actually quite low considering the melaena, but it could certainly have gastritis. Is the diarrhoea acute or chronic? [11:51, 29/04/2022] Riaan Mulder: Adrian, no haematology yet, but clinically not anaemic and body in fair condition. The gastritis is of a couple days duration [11:58, 29/04/2022] Adrian Tordiffe: No vomiting or regurgitation? [11:58, 29/04/2022] Riaan Mulder: No [12:03, 29/04/2022] Adrian Tordiffe: Obviously worth doing faecal floats etc...to rule out intestinal parasites, but lymphoplasmacytic gastritis/enteritis very likely. I treat with Amoxiclav for 7 days. Omeprazole to be given only for a couple of days if ulceration is suspected (unlikely if not vomiting and still eating). Diet changes may be indicated to increase the amount of collagen in the diet and feed less frequently (3 to 4 meals per week) [12:09, 29/04/2022] Riaan Mulder: Thx Adrian, I did that, started with Nexium IV, found that works well in acute cases; do you think the penis issue might just be a complication from the diarrhoea? [12:15, 29/04/2022] Adrian Tordiffe: It could just be from straining from the diarrhoea?? I am assuming it was visible prior to immobilization? [12:16, 29/04/2022] Riaan Mulder: Yes it was, we put a temporary mattress suture to protect the tip. [12:16, 29/04/2022] Adrian Tordiffe: 👍🏻
